Before you start completing this form, it is important that you insert your PSA Member Number / PSA ID and the email address you use to access your PSA account on this screen to enable the system to auto-fill your registration details.   

To locate your PSA Member Number - go to https://my.psa.org.au/s/ and login to your account. Click UPDATE PROFILE on the dashboard and your PSA ID / Member Number will be displayed as shown below.
