Following a patient presentation from start to finish, participants will explore how to identify medicine-related problems, prioritise interventions and collaborate with prescribers to optimise therapy. The session will demonstrate how pharmacists can balance evidence-based care with practical decision-making to improve outcomes for patients with complex treatment regimens. Learning objectives: • Discuss comorbidities and the challenges of polypharmacy. • Describe considerations when applying clinical reasoning in complex cases. • Recognise communication strategies that support collaborative care. Competency standards (2016): 1.2, 2.3, 3.1, 3.5
