Synopsis:

Technology has dramatically reduced the likelihood of patient harm through drug selection errors and transcription errors. However, as technology has facilitated more efficient workflows, another form of error has grown. This workshop will identify how pharmacists can reduce the risks of errors, near misses and incidents which cause patient harm from causes of growing risk. Participants will engage in interactive activities that challenge longstanding workflows and practices which can contribute to critical mistakes and support safer patient care.

Learning Objectives:

1. Discuss the potential risks associated with growing causes of dispensing errors.

2. Discuss strategies pharmacists should use to reduce the risk of patient harm occurring from the most common categories of health practitioner errors.

Biography

Peter Guthrey holds the position of Senior Pharmacist, Strategic Policy at the Pharmaceutical Society of Australia. Peter supports policy development, advocacy activities, media, regulatory submissions and practice advice. Peter has driven key PSA policy work, including Pharmacists in 2023, Pharmacists in 2030 and PSA's medicine safety agenda, including PSA's flagship Medicine Safety report series. Peter has over 17 years’ experience in pharmacy policy, regulation, advocacy, quality management and community pharmacy practice. Peter maintains practice as a community pharmacist, working after-hours at Melbourne’s Ascot Vale Pharmacy.
