Polypharmacy is an increasing reality for patients with comorbidities such as diabetes and heart failure. This grand rounds-style session presents a real-world case that highlights the clinical, therapeutic and communication challenges faced by pharmacists supporting patients with multiple comorbidities.
Following a patient presentation from start to finish, participants will explore how to identify medicine-related problems, prioritise interventions and collaborate with prescribers to optimise therapy. The session will demonstrate how pharmacists can balance evidence-based care with practical decision-making to improve outcomes for patients with complex treatment regimens.
