Join RACGP General Practice of the Year 2024 – Growlife Medical co-founder Dr Aaron Chambers, Director of Allied Health and General Practice Pharmacist Lauren Koch and Dietician Jessica Cartwright for a facilitated panel discussion outlining the efficiencies of their practice and the way in which a large multidisciplinary team has been created and is now a sustainable business model, serving the wider community, focusing on population health.

Biography
Jessica has been working as an Accredited Practising Dietitian for almost 4 years at Growlife Medical, and is the Preventative Health Lead of Growlife's Population Health Group. She is a final year PhD candidate at the University of Queensland. She has completed her Bachelor’s degree in Exercise and Nutrition Science, and also holds a Master’s degree of Dietetics Studies.

Biography
Dr Aaron Chambers is a GP and cofounder of Australia's 2024 General Practice of the Year, Growlife Medical.
He’s a digital health innovator and strong advocate for Multidisciplinary Care Teams, having established many new models of care including Allergy First, Telehealth platform GPConsults, and CubCare virtual paediatrics, alongside health system work through his role as GPLO at Children's Health Queensland.
Aaron sits on the RACGP's GP-led Team-based Care Working Group and is the Education Committee chair of the RACGP Queensland Faculty.
Ms Lauren Koch

Biography
Lauren is the Director of Allied Health at Growlife Medical, a role which allows her to lead a diverse team of professionals to contribute to multi-disciplinary patient care. Lauren is instrumental in the population health team that strives to improve the health of the local community through the implementation of patient centred quality improvement projects and health initiatives.
Passionate about optimising chronic disease management for her patients, Lauren performs Home Medicines Reviews for timely medication advice through her work as a credentialled pharmacist.
A graduate of the University of Queensland, Lauren has many years’ experience within Queensland Health hospital pharmacy roles, including the Poisons Information Centre.
Lauren believes in empowerment through education for both patients and health professionals. She has experience in Education and Training roles and has taught evidence-based medicine to pharmacy, nursing and paramedic students at both the University of Queensland and Queensland University of Technology.
Brooke Snow

Biography
Brooke Snow is a pharmacist with a Bachelor of Pharmacy from James Cook University, a Graduate Certificate in Pharmacy Practice, and a Graduate Certificate in Clinical Pharmacy from the University of Queensland. As a Project Manager with the Pharmaceutical Society of Australia, she leads initiatives that advance pharmacy practice and support the integration of pharmacists into multidisciplinary teams. She previously commissioned a pharmacist pilot for Northern Queensland PHN, demonstrating the value of pharmacists in collaborative care, particularly in rural and regional settings. Brooke is passionate about embedding pharmacists in team-based healthcare models.
