This session will explore the concept of person-centred care through a collaborative, hands-on approach to medication management. Participants will be divided into groups, each tasked with tailoring a generic medication list to suit a specific patient demographic, such as those in home care, diabetes management, disability support, or transitioning from hospital care. Through guided discussion, participants will identify key patient features, concerns, and focus areas for medication reviews, ensuring recommendations are relevant to the individual’s needs. The session will culminate in a presentation of each group’s tailored approach and recommendations. Learning objectives 1. Identify the key defining features of different patient groups and how these impact medication management. 2. Discuss the primary concerns and considerations during a medication review, and make person-centred recommendations based on patient context. Competency standards (2016): 1.4, 1.5, 3.1, 3.2
