The activity focuses on enhancing medication safety in Australian aged care and disability sites through onsite pharmacists. By understanding the challenges of high-risk medicine prescribing, the benefits of an onsite aged care pharmacist and pharmacist medicine support in people with intellectual disability, participants will be better prepared to advocate for and implement extended services in their pharmacy practices. Learning Objective: Discuss best practices for managing medicines in people with disability and aged care facilities. Competency Standards (2016): 1.6, 3.1, 3.5

Biography

Dr Amanda Cross is an NHMRC Emerging Leader research fellow at the Centre for Medicine Use and Safety, Monash University. Dr Cross’ research interests primarily relate to medicine safety and quality use of medicines in older people, particularly those at risk of developing dementia or who are currently living with dementia. Dr Cross continues to practice as a clinical pharmacist and is passionate about exploring new roles for pharmacists to support knowledge translation and medication safety in aged care. In 2023 Dr Cross was awarded the Monash University Faculty of Pharmacy and Pharmaceutical Sciences Emerging Early Career Researcher Award.

Learnings from the Implementation of an Onsite Aged Care Pharmacist Position

Biography

Stephanie completed her internship in 2021 prior to becoming a registered pharmacist at The Barossa Pharmacist Nuriootpa in the Barossa Valley of South Australia, which supplies and supports the surrounding aged care facilities and its community. This role led to her becoming embedded as the first onsite aged care pharmacist at Wheatfields residential aged care facility in Freeling for the past year. Since her time at Wheatfields, she has implemented strategies and improved medication outcomes to optimise the health care of each resident. Stephanie is also a third year PhD candidate researching novel oral formulations for HIV medications.

Biography

I am an Accredited Pharmacist currently undertaking a PhD researching pharmacists support in medication management for people with disability at Newcastle University. I have been providing medication review services to people with a disability for the past 6 years. I also own and work in a community pharmacy in Whitebridge, which helps service several aged care facilities and disability group homes. I have been able to use my growing expertise in medication management and governance to support organisations implement safer medication practices throughout every level of their organisation and embed medication safety into their quality assurance frameworks. I stood on the PSA NSW Branch Committee from 2018-2024, serving as NSW President sitting on the NSW Poisons Advisory Committee during this time.
