Chronic kidney disease and heart failure continue to drive significant morbidity, hospitalisation, and healthcare utilisation in Australia, yet many patients remain undiagnosed, undertreated, or suboptimally managed. Clinical audits provide a structured way to uncover gaps in care, identify at-risk populations, and prioritise action within general practice. This session focuses on how audit and population health data can be used to detect missed opportunities in CKD and heart failure management, reduce variation in care, and support more proactive, coordinated management across a practice population. Biography

Sana Issa is an accomplished pharmacist, diabetes educator and business leader with over 35 years of diverse experience across the community pharmacy, general practice, and IT development spaces. Having owned three community pharmacies for 16 years, Sana has a deep understanding of clinical service delivery within the community pharmacy setting. In her current role as Consultant GP pharmacist, Sana is responsible for medication stewardship, clinical governance, quality improvement and risk management. She is passionate about advancing the quality of healthcare delivery and optimising patient outcomes within the primary care setting. Sana has been a driving force advocating for GP-Pharmacist collaboration to enhance patient centred care. She has served as a board member of the Sydney North Primary Health Network since 2023.
