Palliative care relies on coordinated input from multiple clinicians, with effective communication and shared decision-making central to delivering care that aligns with patient goals. This session brings together perspectives from a palliative care pharmacist and specialist physician to explore how care is delivered across the team. Discussion will focus on managing complex symptoms, aligning treatment with goals of care, and navigating transitions across settings. Attendees will gain insight into how collaboration supports safer, more consistent care for patients with life-limiting illness. Biography

Lorna is a pharmacist who has been a part of the Gold Coast Specialist Palliative Care team since 2011, with extensive experience in palliative care both in the UK and Australia. She is passionate about supporting patients and their families to achieve the best care possible through the optimisation of medications for management of their symptoms. Lorna’s focus is on resource development and education, as well as driving new initiatives to develop services now and into the future.
