Collaborative models in general practice are evolving, with pharmacists increasingly embedded within multidisciplinary teams. This panel brings together perspectives across primary care to explore how different models are working in practice, where they are falling short, and what enables meaningful collaboration.
Discussion will focus on shared medication management, navigating professional boundaries, and practical strategies to strengthen communication and trust between disciplines. Attendees will gain insight into how collaborative approaches can improve patient outcomes while supporting more efficient and coordinated care delivery.

Biography
Tracey has been CEO of Inala Primary Care, a charitable, award winning general practice in Queensland’s most disadvantaged suburban location for more than 10 years. The IPC model was named as one of four which should be adopted nationally under the 10 Year Health Reform Process. They have embedded pharmacists in their team of care for seven years. In November 2024, IPC won a Stronger Medicare Award for innovation in healthcare in relation to serving complex patients and regions of disadvantage.
Tracey is Deputy Chair of the Primary Care Advisory Group of the Australian Institute of Health and Welfare. She was a member of the Expert Advisory Panel which in October 2024 delivered to government reports on Practice Incentives and funding for the general practice After Hours Program. She was also a co-author of “Getting Australia’s Health on Track 2024”. To support practices build performance she co-founded Cubiko, Australia’s dominant practice dashboard software solution in 2018. She is co-founder of Co.Design4All which launched in November 2025 to an international audience. In July 2025 she was awarded the President’s Award from the Australian Primary Care Nurses Association for her advocacy for team based care and activities to support health reform in primary care. Tracey is research active and recently released a report with The George Institute for Global Health charting the change strategies required to support practices embrace team based care.

Biography
Dr Danielle McMullen is the President of AMA and a GP in Brisbane. She has been involved with AMA at state and federal levels since 2011, including as President of AMA(NSW). She is recognised leader in health systems and reform, is a board member of the Australian Digital Health Agency and was previously a member of the Strengthening Medicare Taskforce. Danielle has a strong interest in good governance processes and building member engagement with the AMA. She is determined to see a sustainable future for Australia’s healthcare system – both public and private, rewarding careers for doctors, and quality care for patients.

Biography
Dr Michael Wright is a general practitioner (GP), health economist and health services researcher based in Sydney, Australia. Michael combines clinical practice with strategic appointments and academic research analysing the effects of health policy on the quality and performance of primary care. Michael is the President of the Royal Australian College of General Practitioners, Associate Professor at the International Centre for Future Health Systems at the University of New South Wales and is in clinical practice in Woollahra. Michael’s previous appointments include Chief Medical Officer of Avant Mutual, chair of RACGP’s Reference Expert Committee on Funding and Health System Reform, and Board Chair of Central and Eastern Sydney Primary Health Network.
