Chronic kidney disease (CKD) often progresses silently, with many people remaining undiagnosed until the disease is advanced. Pharmacists are uniquely positioned to identify those at risk through routine interactions, medication reviews and recognition of subtle clinical and prescribing clues. This session explores the pharmacist’s role in screening for CKD and identifying individuals who may otherwise slip through the cracks. Through a review of common presentations of advanced CKD, participants will recognise missed opportunities for earlier detection and intervention, and consider how timely pharmacist recommendations can influence patient outcomes.

Biography
Dr Carla Scuderi is the Assistant Director of Pharmacy for clinical services at the Royal Brisbane and Women’s Hospital and is the co-chair of the Queensland Health Statewide Renal Clinical Network and is the chair of the AdPha Nephrology special interest group. A senior clinical pharmacist with 3 decades of experience in many facets of the pharmaceutical industry in both Australia and the UK. Roles include work for Queensland Health as clinical pharmacist, clinical educator and renal team leader, 5 years as an academic detailer for the National Prescribing Service, current Australian Pharmacy Council subject matter expert, Kidney Health Australia clinical advisory committee and she has also worked as a community pharmacist and Home Medicines Review pharmacist. Completing her PhD on the pharmacist’s role in kidney transplant care in 2024, Carla was awarded a Churchill fellowship in 2025.
