Synopsis Effective type 2 diabetes management requires comprehensive oversight from a multidisciplinary team, with the Credentialled Diabetes Educator (CDE) having a core role within that model of care. Come and explore what "a day in the life of a pharmacist CDE" looks like, and how this role supports people living with type 2 diabetes across both embedded primary care and private practice settings. This session offers real-world insight into how diabetes expertise and medication management come together to support collaborative, multidisciplinary person-centred care. Biography

Jaimee is a Pharmacist CDE currently working as a CDE, ACOP and consultant pharmacist in the ACCHO and private space. Through her work in these sectors in rural and remote areas, she has developed a passion for working through the many health and social challenges facing First Nations peoples, with the aim of improving health outcomes. She is also an advocate for providing health care that is person centred, inclusive and driven by connection involving all of the healthcare team. In 2025 she was named PSA Pharmacist of the Year, and the ADEA CDE of the Year for the NT
