Friday, May 29, 2026
9:01 AM - 11:00 AM
Cypress Room 1

Overview

CHAIR: Jo Gross

Details

This workshop will explore the concept of person-centred care through a collaborative, hands-on approach to medication management. Suitable for newly credentialed (or credentialing) pharmacists and experienced consultant pharmacists wanting to explore new roles for consultant pharmacists in a range of multidisciplinary settings. Participants will be divided into groups, each tasked with tailoring their recommendations from one medication list to suit specific patients based on their individual preferences, needs and values and the clinical context in which they are seen. Through guided discussion, participants will identify key patient features, unmet therapeutic goals, and focus areas for the medication review, ensuring recommendations are relevant to the individual’s needs. It will focus on communication strategies, shared decision-making, and improving patient outcomes through a more holistic and collaborative model of care. Learning Objectives: • Identify clinical considerations and personal preferences, needs and values, and how these impact therapeutic goals and medication management. • Discuss priority issues to address during a medication review, and make person-centred recommendations. • Discuss how the clinical setting in which a consultant pharmacist consults with a patient influence communication and collaborative models of care. Competency Standards (2016): 1.6, 2.4, 3.1, 3.5
