Synopsis This workshop will explore the concept of person-centred care through a collaborative, hands-on approach to medication management. Suitable for newly credentialed or credentialing pharmacists and experienced consultant pharmacists wanting to explore new roles in a range of multidisciplinary settings. Participants will be divided into groups, each tasked with tailoring recommendations from a generic medication list to suit a specific patient demographic, such as those in residential aged care, group home care, within an Aboriginal health service, or transitioning from hospital care. Through guided discussion, participants will identify key patient features, unmet therapeutic goals, and focus areas for medication reviews, ensuring recommendations are relevant to the individual’s needs. Participants will also discuss how to review the individual’s immunisation status, identify relevant co-morbidities for consideration, eligibility for subsidised vs self-funded immunisation, and how to facilitate immunisation in specific settings. Learning Objectives By the end of this session, participants will be able to: • Identify features of different patient groups and how these impact therapeutic goals and medication management. • Discuss the primary considerations during a medication review, and make person-centred recommendations on patient context. • Identify recommended and subsidised immunisations and any administration considerations. Competency Standards (2016): 1.6, 2.4, 3.1, 3.5.

Biography

Joanne is a Quality Use of Medicines Pharmacist at the Department of Health Tasmania part-time, an MMR Credentialed Pharmacist who conducts HMRs, and PSA Tasmanian Branch President. Joanne spent her early career building her knowledge and expertise as a clinical pharmacist in regional and metropolitan hospitals across 3 states/territories and in medical information / pharmacovigilance roles in the UK and Australia. During 9 years working as an NPS MedicineWise educational visitor, Jo completed a Master of Public Health, attained the US Board of Pharmaceutical Specialties (BPS) Geriatric Pharmacist credential and moved from Sydney to Melbourne to Hobart where she has planted roots in the shadows of kunanyi (Mt Wellington). Most recently, Jo has kept busy completing a Graduate Certificate of Diabetes Education and supervising two Honours Students for the Monash SPHERE Centre for Research Excellence researching pharmacist's roles in the provision of reproductive healthcare. Jo is also a member of the PSA Consultant Pharmacist Community of Special Interest (CSI) Leadership Committee and the CPC26 Organising Committee
