This panel discussion brings a real world Home Medicines Review (HMR) diabetes case to life through the perspectives of a GP, diabetes educator, and renal consultant pharmacist. Using a complex case involving type 2 diabetes with renal and cardiovascular risk panelists will explore clinical decision making, medication safety, care coordination, and patient centred priorities.
The discussion will highlight practical challenges faced in contemporary diabetes management, the use of available resources across settings, and how collaborative, multidisciplinary care supports safer prescribing and improved outcomes for people living with complex chronic disease.

Biography
Deborah is the current chair of PSA’s Consultant Pharmacist Community of Special Interest and a branch committee member of the PSA Vic. She is a rural pharmacist located in North East Victoria and has a portfolio career consisting of consultant pharmacy, GP pharmacy, diabetes education, advocacy and research.
