Synopsis Chronic kidney disease is a common and often under-recognised condition, with significant implications for patient morbidity and cardiovascular risk. This session will outline the development and implementation of a GP pharmacist–led chronic kidney disease quality improvement program within general practice. The program focused on identifying patients at risk of CKD progression, optimising evidence-based pharmacotherapy, and supporting guideline-aligned monitoring and management. Practical insights will be shared on integrating pharmacists into primary care teams and implementing sustainable quality improvement activities to strengthen CKD management in general practice Learning Objectives - Recognise opportunities for earlier identification and monitoring of chronic kidney disease (CKD) using guideline-recommended screening and risk stratification approaches. - Describe the role of a GP pharmacist in supporting chronic disease management and improving CKD care through quality improvement initiatives. - Identify strategies to optimise pharmacotherapy and implement practical quality improvement activities to improve outcomes for patients with CKD. Optimising Chronic Kidney Disease Care: A GP Pharmacist–Led Program in General Practice

Biography

Shona Oxley is a GP Pharmacist, Consultant Pharmacist, and Community Pharmacist working in Northeast Victoria. She completed her Bachelor of Pharmacy at Charles Sturt University in Wagga Wagga in 2010. Shona became accredited with the Australian College of Consultant Pharmacy in 2018 and completed a Graduate Certificate in Diabetes Education and Management through the University of Technology Sydney in 2025. She is currently working towards credentialling as a Diabetes Educator with ADEA. In her role as a GP Pharmacist at Northeast Family Medicine in Wangaratta, Shona works as part of a multidisciplinary team supporting quality improvement initiatives across general practice. Through Murray PHN-funded projects, she has contributed to the development and implementation of programs focused on chronic kidney disease, heart failure, diabetes, COPD, and osteoporosis management. Shona has also been involved in several collaborative healthcare initiatives including a multidisciplinary frailty event, the rollout of the National Lung Cancer Screening Program, and diabetes quality improvement activities in general practice across the Murray PHN region. Her professional interests include chronic disease management, diabetes care, and strengthening multidisciplinary collaboration within primary care.
