This presentation highlights the emerging role of a non-dispensing community pharmacist at Latrobe Community Health and its early impact on chronic disease management in the Latrobe Valley—an area characterised by significant socioeconomic disadvantage and low health literacy. Commencing in February 2025, this role was established to address gaps in medication management, compliance, and patient understanding that often contribute to poor chronic disease outcomes.

Biography
Clare is a Community Pharmacist for Chronic Disease Management at Latrobe Community Health.
Since commencing in her role in 2025, Clare has been actively providing Home Medication Reviews (HMRs) to patients across the Latrobe Valley. In her role, Clare works closely with general practitioners and other members of the healthcare team to support safe, effective and optimised medication use for people living with chronic and complex health conditions.
