3 back-to-back sessions focusing on diabetes and MDT care. Learning Objectives: • Describe the mechanisms linking type 2 diabetes with progressive kidney dysfunction. • Discuss when pharmacotherapy may be recommended to help preserve kidney function in people living with type 2 diabetes. • Describe the role of a pharmacist CDE in primary health care settings • Identify multidisciplinary team models that support comprehensive type 2 diabetes management • Recognise challenges in delivering person-centred care for people living with type 2 diabetes Competency Standards (2016): 1.6, 2.4 3.1, 3.5, 3.6
