Pharmacists are increasingly being asked to work within multidisciplinary teams in general practice, particularly in areas with high chronic disease burden. This session draws on heart failure care in the Latrobe Valley to describe how pharmacist input was incorporated into routine clinical practice, the challenges encountered, and the impact on team based care. The focus is on practical lessons for pharmacists considering similar work beyond traditional service models.
