This workshop will explore the evolving practice of aged care on-site pharmacists, focusing on their contributions to medication safety and the quality of medicine use. Attendees will learn practical strategies for optimising medicine management and improving resident outcomes through medicine stewardship principles, clinical governance, and quality improvement activities. Through interactive scenarios and case studies, this session will equip pharmacists with practical tools to enhance safety, compliance, and the quality of care in aged care settings.
Learning objectives:
1. Describe the responsibilities and potential impact of an aged care on-site pharmacist (ACOP).
2. Discuss the role of the ACOP in clinical governance and quality improvement activities.
3. Discuss general medicines stewardship principles.
4. Identify actionable recommendations to address medicine use trends and enhance compliance.

Biography
Dr Manya Angley is an Advanced Practice Pharmacist who began her career in academia. She continues to undertake research in medicines safety at care transitions and has a particular interest in improving health outcomes for people with neurodevelopmental disorders.
Manya was the proud recipient of the Pharmaceutical Society of Australia (SA/NT Branch) Pharmacist of the Year (2021) and the Australian Association of Consultant Pharmacy MIMS Consultant Pharmacist of the Year (2021). Manya registered as Positive Behaviour Support Practitioner (PBSP) in 2023 which is an opportunity to integrate her professional and lived experiences as a carer of family members with neurodevelopmental disability. Her current focus is aging in intellectual disability.

Biography
Nerida is a credentialled pharmacist with 14 years experience working in Aged Care. As Operation Lead at Meditrax, she coordinates a team of experienced credentialled pharmacists providing RMMRs and QUM services to aged care facilities and organisations across NSW and interstate. Her favourite part of working in aged care is hearing the stories of so many of our older people followed very closely by the ability to make a difference to the day to day quality of life for residents.
Nerida’s other area of interest is digital health. In 2021 she was awarded an Emerging Leader Scholarship from the Digital Health CRC and completed her Graduate Certificate in Digital Health from RMIT University. She has worked with Central and Eastern Sydney Primary Health Network (CESPHN) as a Digital Health officer and is well versed in digital health tools including My Health Record and telehealth platforms like Healthdirect.
She works with the Australian Pharmacy Council as a subject matter expert and is on the Clinical council for her local PHN. In 2024 she was recognized as a foundation Fellow of the Australian and New Zealand College of Advanced Pharmacy in the areas of Medicines Management and Geriatric Medicine.

Biography
Dr Amanda Cross is a consultant pharmacist in both the community and residential care setting, as well as a research fellow at the Centre for Medicine Use and Safety, Monash University. As a dual clinician researcher, Dr Cross’ research ideas stem from real world clinical issues encountered in her clinical practice and primarily relate to medicine safety and quality use of medicines in older people. Dr Cross is currently exploring new roles for pharmacists to work at a system level to support clinical governance and medicine safety in residential care. Specifically, she is evaluating the role of pharmacists as knowledge brokers to support guideline implementation and knowledge translation. Dr Cross has published 39 articles in scientific journals, and has secured over $8.8million in research funding.

Biography
Oscar is a credentialed pharmacist providing HMRs, RMMRs, and ACOP clinical pharmacy services for over a decade. He graduated from University of South Australia with a Bachelor of Pharmacy and has a Graduate Certificate in Primary Health Care (Diabetes Management and Education) from Flinders University, and Grad Certificate of Clinical Pharmacy (Aged Care).

Biography
Neil is a consultant pharmacist based in Melbourne, specialising in Quality Use of Medicines (QUM) in the residential aged care setting. His extensive involvement in the residential care environment involves reviewing resident’s current medication and providing QUM activities such as education, audits and advice.
Neil has been involved with advising NDIS support providers on Quality Use of Medicine activities including updating medication management practices and developing e-learning medication related resources.
Neil was the recipient of the Victorian Pharmacist Medal in 2014, PSA Quality Use of Medicines in Pain Management award 2016 and Australian Association of Consultant Pharmacist award 2019.

Biography
Raymond is an early career pharmacist and credentialed pharmacist delivering medication management services upon establishing Pill Box Education. With prior experience in Community pharmacy with an interest in the implementation of pharmacy services (Administration of medicines by injection, immunisations, MS-2 step, take home naloxone, UTI services and OCP renewal services), he is now currently practicing as an aged care onsite pharmacist (ACOP) with Pennwood Aged Care services and as a practice support pharmacist with the Pharmaceutical Society of Australia.
Raymond has been the proud recipient of the PSA SA Early Career Pharmacist of the Year (2024), Chemist Warehouse SA Intern of the year (2020), PSA SA gold medal (2019) and SA/NT Branch student research award (2019).
