This session explores the heightened risk of medicine related harm faced by First Nations People during transitions of care, examining systemic and contextual factors through culturally sensitive case studies.
It highlights the vital role of medication management-accredited pharmacists in reducing these risks by collaborating with patients, families, and healthcare teams in a culturally safe manner to ensure Medication Safety for First Nations People.

Atinuke is a credentialed pharmacist and diabetes educator at the Victorian Aboriginal Health Service (VAHS), where she has worked for over a decade. With extensive experience in Aboriginal Community Controlled Health Organisations (ACCHOs), Atinuke is deeply committed to advancing health equity and improving access to care for First Nations People.
