Explore the growing prevalence of multimorbidity, focusing on its epidemiology, risk factors, and how pharmacists manage patients with multiple chronic conditions. It will examine the primary risk factors contributing to multimorbidity, such as age, lifestyle, and socioeconomic influences. The potential for pharmacists to address these challenges will be highlighted, mainly through medicine management and patient education. The session will also cover evidence-based interventions for managing multimorbidity and integrating pharmacological and non-pharmacological approaches.
