Through a real-world case study, this session explores how GPs, pharmacists and aged care staff can work together to deliver coordinated, person-centred care. It will examine information handover processes, medicine reconciliation following hospital discharge, and practical enablers and barriers to shared care. The discussion will highlight future directions for co-located and integrated care models that strengthen communication, continuity, and medicine safety across the aged care system. Learning objectives: · Describe effective communication and handover processes between pharmacists, GPs and aged care staff. · Discuss the pharmacist’s role in medicine reconciliation and follow-up after hospital discharge. · Identify practical enablers and barriers to implementing integrated aged care models.
